How should I make the door schedule in this form on the drawing?

The upper row is only for ASD, FSD, SD statement types.
The second is only for PD statements
The third is to list only WD and WPD.
1. How do I make only the front part? (Header)

2.I need to create an empty space first

3. How to divide by 5 x 3

We used to display our door schedules by door style/type and give a total of each door type with all of its parameters in a massive matrix schedule on an A1 drawing. We then generated a separate schedule just showing an elevation of each door type. This was problematic and prone to error.

We now show every single door with an image of it and all of the parameters in an A4 or A3 document which auto splits across a multi-page layout sheet.

I’d be happy to send you an example of both for the same project as we switched during that particular project.
