Revision History Object

I'm new to using the Revision History Object. I have added it to the master plan layout, have linked all changed to the appropriate wall etc but changes will not show in this table, even though I can see them in the project Indexes change list. 

Have searched high and low for a solution and no doubt I am missing a step but have no idea what it is. Hoping someone can assist as I need to issue the documents today

thanks - solved issue with IT support. I had put the object in the title bar rather than the master layout. Knew it would be simple but talk about wasting so much time looking for a solution over the weekend

Thanks, have looked at this but will re assess. The issue is not showing up on the revision history object, they are obviously not linking to each other

Do also note that replacement of the word "Issue" with the word "Transmittal" has created a fair amount of confusion of late.
