project library loads things twice...

Just a quick question to see if this is happening to other people or if it's just us...
We've got a project library for each project, which lives with the project file on the server. When we create project specific object (patches, stairs etc) they are saved in this project library folder. If at a later date we open the object and edit it, the next time the project is opened and AC loads its libraries, the custom made objects seem to be loaded twice: once with the folder project library and once again on their own...
This is not causing any problems as such but also it shouldn't be happening.
Any ideas what 's wrong?

(if i'm not being clear, let me know and i'll post some screen snaps of the loaded libraries and duplicated objects in the report thingy)

Thanks for your reply... This is what we have loaded in our projects
From LOCAL hard drive: ONLY AC standard library, nothing changed/modified in there... EVER....
From SERVER:
1. office standard library - this is a folder with a bunch of .gsms which are standard 2D stuff that tend to be common in our projects - rarely ever modified
2. Project specific library - again this is a folder that starts it's life empty and as the project grows, .gsm objects are added to it like stairs from stairmaker, 2D patches (made with marquee), a keynote label object with NBS spec notes...etcetc.

With the above in mind, I'm not sure what you mean by 'local project library'.

Typical library manager window:
all of the folders seen in the right-hand pane except the archicad standard library are on the server... and they are folders, not containers.
I neglected to mention the texture library and furniture library earlier... these are rarely ever modified, the texture one is used for materials' assignment, the furniture contains 2D .gsms

I tried to replicate what I've been describing and I now have a new phenomenon with stairs only.
this is what I did:
placed a stair on plan from the project specific lib. Saved as test stair in same folder. Opened it with stair maker and modified a setting. Saved it and closed the file and archicad. Reopened. This is what I got:

The stair I used to modify was one of the two above my test stair (not sure which one, but surprised to see them both in the report)
no idea what just happened.
