Strange Modal Ghost Dialog Box

A strange dialog box with nothing in it has begun appearing on my screen. Does anyone know where this is coming from or how I might have activated it? It's really annoying as I can't touch anything behind it without dispacing the main AC window.

Seems to be an issue with Tahoe.

I am experiencing production challenges with the following:

1. Icons - see attached image with partial icons and text showing in bars. Is there a fix or workaround?
2. Menue - for whatever reason a residual image/actual menue with not selection remains behind other selections which requires me to restart the session. Is there a fix or workaround?

This is occuring using the "29WE "Architectural Workspace 29"

Any thoughts or vacines are welcome.  Thanking the Community ahead of time.
