Hi,
So I have installed the accessories Lib. pack file for 28 and can see it in the object library, although all it does is install that weird shape with a hole in it that seemed to plague me in the 27 version. I have checked Add-on Manager, and it's in there as well. Screenshot attached, but no matter how many times I've tried to reinstall and restart Archicad, I have no design extra tools in the Design Tool bar. Before I go completely insane, has anyone else had this problem? The last resort is to reinstall Archicad completely, but honestly, I would rather not have to!
Any ideas? Thank you, you would literally save my Friday deadline.

If the accessories add-on is installed (it seems to be OK) and you have loaded the accessories library pack, then all should be OK.
But if you have a custom Work Environment, the commands might not be in your menus.
Try switching to a default WE or you can amend your personal WE to add the commands to any menu that you want.
I would look at that before you do a complete re-install.

Barry you have literally saved my weekend you are such a star! So I switched to Architecture Profile, all set to standard and then I added the design tool into the design toolbar and voila! I can breathe, thank you!
