Not exactly sure of the answer you are seeking - but here is my take on the need to place a title on each page of a layout book:
On the Master Layout, place an autotext location of the required info from the Book Info menu dialog. You may need to set the various justification points to get the title to be in the place you want. For the Scale Item, It needs to be a Lib part (I think) which would require you to place the thing in AC and suffer the pangs of wondering if you got it in the right place. If you did not need a graphic scale but wanted a scale note, you could use the auto title in PM and place it after locating the individual PMK or linked files.
All of this sounds complicated but is quite simple if you plan the placement condition as part of the master layout and title block.

hmm..the "title" I am using is actually a Title in AC, and as such is appended to the imported drawings. The problem I have found is that although I can specify the location for the title appear at (eg lower right)--and the title always hus up to the outside of the drawing--since the size of my drawings change from page to page (floor plan v elevation and so on), the location of the Title on the page also varies. My solution to this is to put a hotspot on my Title, and another on my Master ,and then move the Title to the right spot. Works OK, but it can be a little mysterious as to how to grab ONLY the Title, without grabbing the whole shebang. Hints on that would be great, as there is no "magnet" selecc tool in PM, as far as I can tell.
Thanks..!

I have been using the autotitle in PM to some success - but without the graphic scale. I am going to try another title type to be placed with the view that includes a graphic scale as this is a great way to make the individual views better - so will report later.

Zobi:
The Title has 9 selection points, so you can select only the Title by shift-clicking on one of these points when using any tool. Also, if you swipe a rectangle using the Arrow Tool only across a portion of the Title (and not the Drawing) only the Title will be selected.
