What is the Reveal Depth value set for the Door? If it is set too high it can cause this phenomenon.
Also, whether you set it to be measure from the Wall Face or the Wall Core can also influence where it is placed.

It is a known issue (maybe just with the USA library?).

The latest library update is supposed to fix it.
Or if you read the post before the one I linked to above, you can try changing the wall closure.
