Doors showing disconnected from wall

I'm having a problem where the doors are just showing slightly off from the wall. Sometimes I can work around this by rotating, mirroring, and/or flipping... but that's cumbersome and I just want to fix the root of the problem. Anyone know why this is happening or is it a glitch?

What is the Reveal Depth value set for the Door? If it is set too high it can cause this phenomenon.
Also, whether you set it to be measure from the Wall Face or the Wall Core can also influence where it is placed.

It is a known issue (maybe just with the USA library?).

The latest library update is supposed to fix it.
Or if you read the post before the one I linked to above, you can try changing the wall closure.
