Creating an A4 document transmittal in archicad 29 using index's

Sorry if this has already been asked.

I am trying to create an A4 document transmittal for our drawings sheets in columns in this order.

Layout ID, Layout name, Layouts scales. Then the following columns showing the revision history for each sheet. Apparently archicad 29 (and maybe earlier) doesn't have current revision date and current revision description in the fields/ transmittal history - add fields. This is the way AI told me I can make my transmittal.

Has anyone other ways I can do this. Example
