ui_dialog for custom MVO page

I am trying to give a title to my custom model view options page but it doesn't seem to change the title of the page. I have been trying to use the ui_dialog function which seems to be as basic as it gets but it doesn't seem to change. Any help is appreciated. Thanks

If you post the code for the Parameter Script that includes the UI_Dialog, maybe someone can spot the issue.
