Garage Door Opening with Angled Corners

I am looking to model a Garage Door Opening with Angled Corners. The Upper left and right corners of the garage door are not at the normal 90-degrees, but they have a 45-degree fillet.

Any help would be appreciated.

I use D3 Smartparts which have this option..

I was not interested in the Smartparts at this time. I looked on Objects Online and did not see "just" a Garage door.
