Please see attached for duplicate items. I may have used this image once in a late night rendering test session but cannot be sure. Any ideas why I might be getting duplicate of just this one item and why in the .pla and .lib files?

Because you are loading PLA and LIB at the same time.
You can open the PLA, then resave it under the same name, this time not enabling Background Image to be saved with the Archive.
Then reload Libraries and it should be fine.
