I place a floor plan reference on my layout (reference is pink). I set the pdf in the publisher set to "print reference". Everything looks great on the layout in the publisher set but in the pdf the doors and windows of the reference are missing. I have attached a screen shot from the publisher set and from the pdf.

Interesting issue.
The lines and fills of the trace also seem to be publishing darker and thicker.
I don't know anyway to change this.
However, instead of using a trace reference, why not just overlay the actual view you are using for the reference.

I tried restarting. I also tried deleting the layout and recreating it and neither worked. I just tried the same process in another project file and same thing, the windows and doors are missing, but on this other project, it did not darken or thicken the reference.
when you say "overlay the view": you mean take the reference view an put it on top of the other view on the layout? with no control of color or transparency I don't see how that is a work around. I guess not enough users publish references on layouts but I have always had trouble getting it to work.
